Tooth Extraction Aftercare: Do's and Don'ts (First 3 Days)

Bite on gauze for at least 30-45 minutes at a time. Keep changing to a fresh gauze pad until you finish the full pack given by the clinic.

Check you’re not allergic to the medicine given by the nurse before taking it.

Do not gargle or spit out blood for 48 hours after surgery. Blood needs to stay in the extraction site for a clot to form – this clot is what protects the wound while it heals.

Brush your teeth as normal, but gently around the extraction area. Just let the water flow out of your mouth – don’t rinse or gargle hard. Clean mouth = faster recovery.

Swallow your saliva and keep the extraction site as dry as possible so the blood can clot.

Keep your fingers and tongue away from the surgical area – poking at it disturbs the clot.

👶 For kids: Don’t let them bite or play with their lip while it’s still numb – this can cause a painful ulcer the next day.

Managing Swelling

If swelling appears:

❄️ Use an ice pack on the cheek/face next to the surgical site.

⏱️ 15-20 minutes on, 10 minutes off, repeat until you go to sleep.

Eating & Drinking

🚫 No straws – the suction can dislodge the blood clot and cause dry socket.

✅ Stick to soft, easy-to-swallow foods. Avoid hot or spicy food.

– Normal extraction: soft diet for the first 24 hours

– Surgical extraction: soft diet for 48 hours

General Precautions (First 2 Days)

🚭 No smoking for at least 2 days after surgery

🏃 Avoid strenuous activity or exercise

💊 Take your medication on time, as prescribed

✋ Keep fingers, tongue, and straws away from the extraction site

Day 3 Onwards

⚠️ On the third day:

🌡️ Switch from ice packs to a moist heat compress instead

👄 Start gently exercising your mouth muscles – slowly open and close your mouth

🩸 If bleeding continues on day 3, visit the nearest clinic or hospital

Avoid rinsing or spitting forcefully, using a straw, smoking, strenuous exercise, and touching the extraction site with your tongue or fingers for at least 48 hours. These all risk dislodging the blood clot that protects the wound.

Follow the gauze, diet, and hygiene instructions closely, keep the site clean by brushing gently elsewhere in your mouth, avoid smoking, and attend any follow-up visit your dentist recommends. Good clot protection in the first 48 hours is the single biggest factor in fast healing.

Day 3 is when initial swelling often peaks and is also the highest-risk window for dry socket (when the blood clot dislodges or dissolves too early, exposing bone and nerve). If pain is worsening rather than improving by day 3, or bleeding persists, visit a clinic promptly.

Not in the first 24 hours – any rinsing, including salt water, risks dislodging the clot too early. From day 2-3 onwards, a gentle warm salt water rinse (without forceful swishing) can help keep the area clean once the clot has stabilized.

Most patients can return to light daily activity the next day, but should avoid strenuous exercise or heavy lifting for at least 2-3 days. Surgical extractions (e.g. wisdom tooth) may need a few extra days.

Yes – there’s no restriction on talking or phone use. Just avoid activities that involve sucking, blowing, or spitting motions (straws, whistling, smoking) during the first 48 hours.
